CHANCE Program
(Children Having Access to a Nurturing, Compassionate
Environment)
Ponte Hall
The CHANCE
Program at Ponte Hall is a short-term, seven-day program for boys age
10 to 16 who have been discharged from psychiatric hospitalization, but
still in need of treatment before returning home. Ponte residents stay
from 3 to 6 months, attend school in the Campbell County school system,
and have the opportunity to work on personal control, family issues,
and behavior changes across settings – including
school, home, and community. Parents and guardians of these young men
join the family therapy team to enhance communications and understanding,
and to tackle problems as a team. The program also includes a comprehensive
90-day home-based aftercare component.
Ponte Hall Admission Criteria:
- adolescent boy age 10-16 with severe, persistent Axis
I diagnosis other than dementia or substance abuse
- discharge straight to Campbell Lodge Boys' Home from
psychiatric hospital
- IMPACT Plus certified – CHANCE
Program staff will work with discharge planners and workers to complete
required application paperwork to IMPACT Plus and to facilitate successful
transition
